CSO Fall-140 Roster of Religion Teachers

Instructions:

1. Download Excel File form and complete fields, then save completed form.
2. When you save form, make sure to add the name of your school to the beginning of the file name, like this:
   “YourSchoolName-CSO-FALL-140.Religion-Teacher-Roster.2024-25.xlsx“
3. In the Submission Form below, enter your name, email and school.
4. Click on “Choose File” button, select file to upload and click “Open” button to attach completed file.
5. Click “Submit” button to submit form.
